Same-Day Dental Implants in the Bay Area: Are You a Candidate?

Reviewed by Dr. Na Zhao, DDS, PhD — Harvard School of Dental Medicine · UCSF School of Dentistry · Last reviewed April 2026

Direct Answer

Same-day dental implants — clinically called immediate-load or immediate-placement implants — allow a crown or provisional bridge to be attached to the implant on the same day as surgery, rather than waiting three to six months for osseointegration. At Dr. Na Dental in San Mateo, approximately 40–50% of single-implant candidates qualify for same-day placement based on bone density and the absence of active infection. The procedure costs the same as a conventional implant: $4,500 to $6,200 at our practice.

Immediate Implants vs. Conventional Implants

Note: cost is identical — you pay for the implant, abutment, and crown regardless of timing. The "same-day" option is about your timeline, not a premium add-on.

Who Qualifies for Same-Day Implants?

Qualification requires meeting all four criteria:

1. Sufficient primary stability The implant must achieve an insertion torque of ≥ 35 Ncm at placement. Dr. Na measures this directly with a calibrated torque wrench during surgery. This confirms that the implant is mechanically locked into the bone even before biological fusion begins.

2. Adequate bone density and volume A CBCT scan is required before every same-day case. We need a minimum bone width of approximately 6 mm at the implant site and adequate vertical bone height. Soft or very porous bone (Type IV bone) is a contraindication for immediate loading.

3. No active infection at the extraction site If you are having a tooth extracted and an implant placed in the same appointment (immediate placement), there must be no acute infection. Chronic low-grade periapical infections can sometimes be treated and still allow same-day placement — Dr. Na makes this call based on the clinical presentation.

4. Good systemic health Uncontrolled diabetes, active chemotherapy, bisphosphonate therapy for osteoporosis, and heavy smoking are all relative or absolute contraindications. Well-controlled systemic disease with medical clearance often allows same-day placement.

Who Is Not a Good Candidate?

You are likely NOT a candidate if you have:

  • Active gum disease (periodontitis) that has not been treated
  • Bone loss requiring a bone graft (graft must heal first, 3–6 months)
  • An acute dental abscess at the implant site
  • Uncontrolled blood sugar (HbA1c above 8%)
  • Bruxism (severe teeth grinding) without a night guard — excessive bite forces can destabilize an immediately loaded implant before it integrates

In our practice, patients who don't qualify for same-day loading still receive their implant that day — we simply delay the crown attachment until osseointegration is confirmed, which adds a few months to the timeline but does not compromise the final result.

The Same-Day Implant Procedure at Dr. Na Dental

Step 1 — AI-Assisted CBCT Planning (Pre-visit or Day of)

Dr. Na analyzes a cone-beam CT scan using AI-assisted planning software to map the exact implant position, angle, and depth before the first incision. This digital pre-planning is what makes same-day placement safe: the drill guide is fabricated from the CT data so there are no surprises during surgery.

Step 2 — Extraction (If Applicable)

If a failing tooth is present, it is extracted gently to preserve the bone walls of the socket. A socket that retains its walls is an ideal same-day implant site.

Step 3 — Implant Placement

The titanium implant is placed with a calibrated insertion torque of ≥ 35 Ncm. If this torque is achieved, same-day loading is confirmed.

Step 4 — Crown or Provisional Attachment

A temporary crown or bridge is attached and adjusted so that it is slightly out of contact with opposing teeth ("out of occlusion"). This is critical — the provisional must not bear full bite forces during the healing phase, or the implant can fail.

Step 5 — Osseointegration and Final Restoration

Over the next 3–4 months, bone fuses to the implant surface. At a follow-up appointment, the final custom crown is placed.

Same-Day Implants for Front Teeth vs. Back Teeth

Same-day implants are most commonly placed in the front of the mouth (incisors and canines) for two reasons:

  1. Front teeth experience lower bite forces than molars, reducing the risk of micro-movement that can disrupt osseointegration.
  2. Aesthetics matter immediately — a missing front tooth is socially visible in a way a missing molar is not.

For molars, immediate loading is possible but requires higher bone quality and stricter torque thresholds. Dr. Na recommends case-by-case evaluation via CBCT.

Recovery After Same-Day Implants

TimeframeWhat to Expect
First 24 hoursBleeding, swelling; cold pack every 20 minutes; rest
Days 2–7Soreness peaks and subsides; soft diet; no hard or crunchy foods
Weeks 2–12Osseointegration; avoid biting hard foods with provisional crown
Month 3–4Final crown placed; full bite load restored

The provisional crown is for aesthetics and protection — do not bite hard or sticky foods with it. Your bone is still integrating during this time.

Why Same-Day Implants at Dr. Na Dental?

For Bay Area professionals who cannot afford weeks of wearing a removable flipper or partial denture, same-day implants restore appearance and confidence immediately. Dr. Na's AI-assisted surgical planning — a skill she developed through continuing education including coursework from DeepLearning.AI and clinical research training at Harvard — ensures that the CBCT analysis driving the drill-guide design is as accurate as the technology allows. This pre-surgical precision is why same-day loading is predictable at our practice rather than a gamble.

Dr. Na Dental serves San Mateo, Foster City, Burlingame, Palo Alto, Mountain View, and all Peninsula cities. Appointments are available on weekdays and selected Saturdays.

FAQs

How do I know if I qualify for a same-day implant?

You need a CBCT scan and a clinical exam. Dr. Na will measure your bone density, check for infection, and assess your systemic health. The entire assessment takes about 60–90 minutes and is included in the consultation fee.

Is a same-day implant less reliable than a conventional implant?

In patients who meet the clinical criteria, same-day implants have success rates of 94–97%, compared to 96–98% for conventional implants — a small and clinically acceptable difference. Patient selection is the key variable.

Can I get a same-day implant if I have a tooth extracted the same day?

Yes — this is called "immediate placement" and is actually one of the best situations for same-day implants because the socket is fresh and blood supply is optimal. The key requirement is no active infection.

Will the same-day crown look natural?

The provisional crown placed same-day is a temporary restoration and may not perfectly match surrounding teeth. The final custom crown, placed at 3–4 months, is shaded and shaped to match your natural dentition precisely.

What happens if my implant doesn't achieve 35 Ncm torque during surgery?

Dr. Na will place the implant but leave it submerged (without a crown) for the standard 3–6 month healing period before loading it. The outcome is the same; only the timeline changes.

Does same-day implant cost more?

No. At Dr. Na Dental, the fee for the implant, abutment, and crown is the same regardless of loading protocol. You may pay for an additional provisional crown if one was not originally planned.
